I've bought a few CDs based solely on reviews (Elliott Smith probably 
being the latest).  Usually I can't get into a review until after I 
already own the CD.

I find myself lately buying stuff based on cover and booklet art, 
especially local stuff.  I peruse the local section, look for the nicest 
covers, then peruse the CD booklet...  I've gotten some pretty good 
things this way (Mystery Wagon, Radiolaria, Nez Peach).
